Homeless History Tracking Form

The Excel Homeless History Tracking Form (HHTF) is meant to allow homeless-service providers to accurately and consistently document clients’ length of time homeless according to the parameters set forth by HUD’s “Chronically Homeless” Final Rule. The form is housed in an Excel workbook that also contains a Key and two example tabs. The HHTF will open as a Google Sheet.

If you have Google Drive:

You will want to download it by going to FILE, hover over DOWNLOAD and selecting MICROSOFT EXCEL.
From here, you will be able to save as and edit the document as needed.

If you don't have Google Drive:

Click the Download icon
This will download and then you should be able to open it as an Excel
From here, you will be able to save as and edit the document as needed.
