[OUTS-102] Performance Monitoring

Report Purpose and Summary 

This report measures program performance, and enables outside entities to apply scores/grades to agency programs. It is intended to be used for the NoFA competition and can be used in comparing program performance.

Instructions

Make sure you are operating in Clarity under the agency you are running the report for. You can check and change this under your name in the top right corner.

Step 1: Click on the Launchpad, then click on the Reports icon. 

Step 2: In the Report Library, click the down arrow next to the Program Based Reports folder to expand the section.

Step 3: Click the Run button next to [OUTS-102] Performance Monitoring.

Reporting Parameters

Step 4: Enter the following parameters

Program Type(s) Select the program type to filter the choices available in the Program(s) parameter (hold either control/command/shift button to select multiple)
Program(s) Select the program(s) to include in the report (hold either control/command/shift button to select multiple)
Report Date Range Select the date range of enrollments to include in the report
Report Output Format Select *Web Page, PDF or Excel

*Drilldown Functionality is available when run in Web Page Format 

Step 5: Click submit

The file will be sent to the Report Queue. Once the report is processed, you will receive a notification that the report is ready.

To open the report, either select Open on the popup or click the Queue icon and then click Open.

Reporting Details

This report provides information on four categories of performance:

  • Project serves “harder to serve” homeless populations
    • This section captures the percentage of households served who reported at least one of the criteria listed below:
      • Mental Illness
      • Alcohol Abuse
      • Drug Abuse
      • Chronic Health Condition
      • HIV
      • Developmental Disabilities
      • Physical Disabilities
      • Domestic Violence
      • Unaccompanied youth under age 18
      • Unaccompanied TAY age 18-24 years
      • % of households meet any one criteria above
      • % of households meet any two criteria above
      • % of households meet any three or more criteria above
  • Reduce length of homeless episodes and new and return entries into homelessness
    • This section only pertains to the following project types:
      • Permanent Supportive Housing (disability required for entry)
      • PH – Housing only
      • Transitional Housing
      • PH – Rapid Re-Housing
      • Street Outreach
      • Services Only
    • The section includes the following information:
      • (PH programs only) % of persons in PH over six months
      • (TH and RRH programs only) % of persons in TH/RRH exited to PH
      • (TH programs only) Average days from entry to exit in PH
      • (SSO programs only) % of households maintaining PH for at least 6 months
        • This information will only pull into the report if data is being collected on the follow-up screens for SO and SSO programs.
  • Increase jobs, income and self-sufficiency
    • This section displays data pertaining to both Cash and Non-Cash Income as well as Education:
      • % of adults with increased or sustained employment income
      • % of adults with increased or sustained other cash income
      • % of adults who increased or sustained mainstream or non-cash benefits
      • % of persons with improved education
        • Note: this will only pull data if at least one of following fields are collected on the program enrollment and status update screens:
          • Currently enrolled in school (education_enrolled)
          • Highest level of school completed (education_level)
          • Earned degree (education_degree)
  • HMIS Participation and Data Quality
    • This section displays data quality information for the reporting period and includes the following:
      • % of persons who exit to known destinations
      • % of persons with income data updated at least annually
      • % of program data response = Don’t Know or Refused
      • Program Entry Average Data Entry Timeliness in Days
      • Program Entry Average Data Exit Timeliness in day

Note: This report can be generated for multiple programs at one time, but it does not categorize results by program. The totals provided are conglomerate scores of all programs selected in the parameters.
