Transitional Housing

All materials developed by ICA are based on the Bitfocus Clarity HMIS platform and reflect the most current information available to us. Materials are updated as new information, guidance, and functionality are released by the vendor. Because the system continues to evolve, there may be updates or changes to the interface or functionality that are not yet reflected in these materials. ICA will continue to review and update these resources as additional information becomes available from Bitfocus.

Transitional Housing programs provide temporary housing and supportive services to individuals and families experiencing homelessness as they work toward obtaining and maintaining permanent housing. Within HMIS, Transitional Housing programs use the system to document participant enrollment, assessments, services provided, case management activities, referrals, housing plans, and program outcomes in accordance with federal, state, local, and Continuum of Care (CoC) reporting requirements.

Transitional Housing is designed to offer a stable living environment while participants address barriers to permanent housing. Programs often provide supportive services such as case management, employment assistance, life skills development, education support, behavioral health services, and connections to community resources. The goal is to help participants increase self-sufficiency and successfully transition into permanent housing.

HMIS serves as the primary system for tracking participant enrollment, service delivery, case management activities, referrals, housing progress, and program outcomes. Accurate and timely data entry ensures communities can measure the effectiveness of Transitional Housing programs and evaluate progress toward reducing homelessness and increasing housing stability.
