Supportive Services programs provide assistance and resources to individuals and families experiencing homelessness or housing instability to help them achieve greater stability, self-sufficiency, and improved well-being. Within HMIS, Supportive Services programs use the system to document participant enrollment, assessments, services provided, case management activities, referrals, service plans, and program outcomes in accordance with federal, state, local, and Continuum of Care (CoC) reporting requirements.

Supportive Services programs focus on addressing barriers that may impact housing stability and overall quality of life. Services may include case management, employment assistance, benefits enrollment, life skills development, education support, behavioral health services, transportation assistance, financial counseling, and connections to community resources. The goal is to help participants access the support they need to improve their circumstances and achieve their individual goals.

HMIS serves as the primary system for tracking participant enrollment, service delivery, case management activities, referrals, progress toward goals, and program outcomes. Accurate and timely data entry ensures communities can measure the effectiveness of Supportive Services programs and evaluate participant progress and service impact.
